NHS Cornwall and Isles of Scilly can only respond to requests for the information it holds. The organisations involved in the integrated care system remain responsible for responding to requests for information they hold and not considered to be held by NHS Cornwall and Isles of Scilly.

Please check our disclosure log before you send a freedom of information (FOI) request. We may have already answered it for someone else and you can find the answer you need here.

What is a disclosure log?

The disclosure log is the public record of information requests we’ve received and responded to. This includes requests made under the Freedom of Information Act.

Information provided under the act may be reused in accordance with the Open Government Licence. NHS Cornwall and Isles of Scilly must be acknowledged as the source. You should also provide a link to the Open Government Licence.

You may:

  • copy, publish, distribute and transmit the Information
  • adapt the Information
  • exploit the Information commercially and non-commercially for example, by combining it with other Information, or by including it in your own product or

This licence does not grant you any right:

  • to use the information in a way that suggests any official status
  • that the information provider and/or licensor endorse you or your use of the information
