Long term conditions survey
We’d like to hear about your experiences and what matters most to you when it comes to your health, wellbeing and support. If you have lived experience of:
- heart conditions
- lung conditions
- diabetes
Please take a few minutes to complete our survey and help shape future services.

Summer 2026: Pharmacy first ambassadors
As summer 2026 got underway, NHS Cornwall and Isles of Scilly and NHS Devon put local people, including members of the long term conditions expert reference group, at the centre of their seasonal health campaign. A network of Pharmacy First Ambassadors was launched to help spread the message across communities.

Summer 2026: Heat health films
Members of the group volunteered to feature in short films for social media promoting heat health advice for people with long term conditions. Watch Paul’s film here:
August 2026: Happy feet
Working in partnership with Cornwall Partnership NHS Foundation Trust podiatry services and members of the long term conditions expert reference group, we launched an educational video to help people living with diabetes look after their feet and prevent serious complications.

Winter 2025: Winter advice on long-term conditions
A central part of our winter campaign 2025 included a series of 5 winter advice films for people living with long term conditions.
These films were developed with the support of several members of our long term conditions expert reference group:
December 2025: 12 days of Christmas
The winter advice films created by members of the long term conditions groups were also featured in our 12 days of Christmas series of winter tips.
June 2025: BSL diabetes films
We worked with Diabetes UK, Hearing Loss Cornwall and members of the deaf community to produce a series of educational films in British Sign Language (BSL).
The films aim to improve understanding of diabetes and help deaf people navigate healthcare services with more confidence.
June 2025: Diabetes walk
Members of the lived experience group took part in a community walk at Lanhydrock, providing an opportunity to connect with others, promote the benefits of physical activity, and highlight the importance of supporting people living with diabetes across Cornwall and the Isles of Scilly.
